I have a loud clicking noise coming from under the hood. It accelarates with rpm. What is it and should I worry about it? Truck is a 97, 3.9L, 75k miles.

I'm not too sure what noise you're getting, but mine makes noise at idle or whatever rpm you want. Fan seems fine though.

I would suspect an exhaust leak from what you are saying. Wait until the motor is cool. Start it and feel around the exhaust manifolds for a leak. You should be able to tell which side it's coming from. If it's not an exhaust leak, try to track down the noise. If it does it at idle, should be fairly easy to pinpoint it.

Oil level is fine as I replaced it not too long ago.

I'll check the exhaust manifold and see if that's the problem.
