Found this free site- pretty cool for keeping records of oil changes, etc. You can enter comments about services and pretty much tailor it to your own preferences.

I made up a spread sheet from excel (microsoft office) and it works great for me, I keep track of my oil changes, along with any other maintenance, I have one for gas, and I also have one for mods and mishaps (flat tires etc..)

I bought my Dak on autobytel.com and they continusly send me reminders. I ignore them. In microsoft works there is a pre-made spreadsheet for auto maintnence, that I use. I started it with the cost of the truck and every thing I put into or onto it I log. It gives me a running cost of my truck minus fuel.
