While I didn't add it for any MPG gains, I did add an ARE tonneau cover last November. I really wanted something to cover my bed so I could keep stuff back there and not have it get wet. I Have noticed MAYBE 1 mpg gain since I got it. Again, I got it more for the storage value, but it it supposed to be the most significant thing you can add to the bed to reduce drag.

Remove the tailgate from the truck when it is not needed. I got 1.5MPG from that with my 4.7L, 5-spd, 3.55 Quad with the T&H package on it.
Lowering (slamming)the truck should help also. Reduces the effective frontal area of the truck.
I have seen trucks with tonneau covers that slope down from the top rear of the cab to the bed level and "fair" in the back of the cab and bed. One of those may help, also.
Narrower tires will reduce the frontal area.
You might replace the cab mounts with steel ones, rather than rubber. Then weld up the bed to the cab, if you really want to take it that far. Make a one-piece body and that would reduce the parasitic drag from the crevice between the two.

Here is a website that shows some mods that a college made to a Dodge Ram and it shows the % increase or decrease in drag for each one of the mods. It is worth checking out as it has some really surprising results on it.
