Subject: 1 headlight/1foglamp no power

My 2000 Dak Quad won't power up the driverside head lamp and the driver side foglamp.

History...headlamp went out, chime, alarm system, seatbelt warning and keyfob all malfunctioned together. Changed out the CTM with same part number (used). Everything working again the but fobs (they need dealer flash) and headlamp/foglamp on drivers side. I looked at the service manuals and it refers to TWO fuses for the headlamps (one for each). I can not for the life of me locate them! Can't find them based on wiring diagram either.
Nothing in the underhood box or the drivers side dash box indicate headlamps.

It goes against my nature I'm but sooo tired of banging my head on the side of this truck....please help this little ol' man;)

Correction to original post...
Both foglamps do work correctly, its only the driver side headlight that's out now. (yeah, I changed the bulb). Chased down a couple of more diagrams and even a picture that shows a supposed hidden fuse block for this vehicle under the dash... none to be found on this slt quad cab.

So, only question remaining is- where in the heck is the fuse/circuit breaker for headlight(s)?
Nothing marked "headlight" (or anything close) under the hood at the power dist box or the fuse box on the left side of the dash. Its got to be a fused circuit, right???
