Subject: '98 V6 Misfire all Cylinders

I've had this problem for a while and can't seem to figure out the cause to fix it, neither can my mechanic.

I have a '98 3.9 V6 with ~135k miles and the check engine light is almost constantly on showing a misfire in all 6 cylinders. The truck runs good overall, does not idle rough or anything. The problem started about 2 years ago when the truck wouldn't start and the crankshaft position sensor went bad and was replaced. The CE light would come on some, but not a lot and would blink at times. It showed a misfire in cylinder 1, so I changed plugs, wires, cap, rotor. The problem continued and then showed misfire in #1 and #3. Checked vacuum lines and noticed an issue, corrected, but problem continued. Misfire then showed in #1, #3 and #5. Now all 6 cylinders show to be misfiring. Mechanic has checked for pressure and all is good, along with plugs, wires, cap and rotor having been replaced. I recently replaced the pickup coil and that did not help any. I have also swithed to 89 octane from 87 thinking that the fuel could be been igniting early, still having issues.

Has anyone else had this issue or heard of it? Any ideas as I'm at a loss?

Check the distributor for wear. Timing chain could be worn too.
A bad/ leaking manifold gaskets, worn injectors also.
