When you turn off O/D, you should have 3 forward gears, Correct? When I have O/D off, it sounds like it shifts through 4 gears. Then I turn O/D on then it shifts into overdrive. Any comments?

by reading a thread on the v8 board I think I figured it out. It's the torque converter locking in third gear.

Yep I got a 46RE (just bought a good used 01 R/T) begging for another Tranny flush and its shifts the same for me.
