It is just the loaded variety, it does not come with a built-in amplifier. The UAW 300W amplifier listed below it is the model I was using to power the Thunderform.

Depends on where you live - because of the size/weight of the Thunderform it will cost anywhere from $35-60 to ship it UPS ground.
