18-19 mpg highway is pretty good. You didn't mention 4x4 or not. Lowering truck, smaller tires and a higher (numerically lower) axle ratio will all help, but don't expect too much more...

I'm only getting 16-17 pm the Hwy. So be happy with it. I had the chance to rent a Ford F-150 supercrew pick-up. I'm not a Ford fan, but its not a bad deal at all to buy one. Got 19 on the HWy and the 5.4 (330) can deffently pull some weight.

Now just what do you have under your hood?? A 440 wedge with a roots blower installed???
I noticed that when the fuel in the tank shifts, the needle moves down to empty but dosn't always come back up. So, if I don't watch it, I'll think that it's a gasoline drunk.
