Question: How can you tell with out a fuel/Air ration gauge, if your truck is running lean or rich? Also, what can running lean or rich cause to happen?

Well, you can check the spark plug tips. Black and lots of carbon and possible wet, you are too rich. Grey and flaky, and most likely you are running lean. With a rich condition you will damage your cat, plugs, and engine(dilutes your oil)as well as get crappy gas mileage. With a lean condition you may damage your piston heads/rings, plugs(due to detonation and preignition)
If it ain't broke, fix it til it is.
